Do not use methylphenidate if you have used an MAO inhibitor such as isocarboxazid (Marplan), tranylcypromine (Parnate), phenelzine (Nardil), rasagiline (Azilect), or selegiline (Eldepryl, Emsam) within the past 14 days. Serious, life-threatening side effects can occur if you use methylphenidate before the MAO inhibitor has cleared from your body.

Do not use this medication if you are allergic to methylphenidate or if you have glaucoma, tics (muscle twitches) or Tourette's syndrome, depression, or severe anxiety, tension, or agitation (methylphenidate can make these symptoms worse).

Methylphenidate may be habit-forming and should be used only by the person it was prescribed for. Keep track of how many pills have been used from each new bottle you receive. Methylphenidate is a drug of abuse and you should be aware if any person in the household is using this medicine improperly or without a prescription.

What is methylphenidate?

Methylphenidate is a central nervous system stimulant. It affects chemicals in the brain and nerves that contribute to hyperactivity and impulse control. Methylphenidate is used to treat attention deficit disorder (ADD), attention deficit hyperactivity disorder (ADHD), and narcolepsy. Methylphenidate may also be used for purposes other than those listed in this medication guide.

What should I discuss with my healthcare provider before taking methylphenidate?

Do not take methylphenidate if you have used an MAO inhibitor such as isocarboxazid (Marplan), tranylcypromine (Parnate), phenelzine (Nardil), rasagiline (Azilect), or selegiline (Eldepryl, Emsam) within the past 14 days. Serious, life-threatening side effects can occur if you use methylphenidate before the MAO inhibitor has cleared from your body.

Do not use this medication if you are allergic to methylphenidate or if you have:

 

bulletglaucoma;
bulleta personal or family history of tics (muscle twitches) or Tourette's syndrome; or
bulletsevere anxiety, tension, or agitation (methylphenidate can make these symptoms worse).

 

Some stimulants have caused sudden death in children and adolescents with serious heart problems or congenital heart defects.

Before using methylphenidate, tell your doctor if you are allergic to any drugs, or if you have:

 

bulleta congenital heart defect;
bullethigh blood pressure;
bulletheart failure, heart rhythm disorder, or recent heart attack;
bulleta personal or family history of mental illness, psychotic disorder, bipolar illness, depression, or suicide attempt;
bulletepilepsy or other seizure disorder; or
bulleta history of drug or alcohol addiction.

 

If you have any of these conditions, you may not be able to use methylphenidate, or you may need a dosage adjustment or special tests during treatment.

FDA pregnancy category C. This medication may be harmful to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant during treatment.

It is not known whether methylphenidate passes into breast milk or if it could harm a nursing baby. Do not use this medication without telling your doctor if you are breast-feeding a baby.

Long-term use of methylphenidate can slow a child's growth. Tell your doctor if the child using this medication is not growing or gaining weight properly.

Do not give methylphenidate to a child younger than 6 years old without the advice of a doctor.

How should I take methylphenidate?

Take this medication exactly as it was prescribed for you. Do not take the medication in larger amounts, or take it for longer than recommended by your doctor. Follow the directions on your prescription label.

Take this medicine with a full glass (8 ounces) of water.

Take this medication at least 30 minutes before a meal. The extended-release forms of methylphenidate (Ritalin-SR, Metadate ER, Metadate CD, Methylin ER, Concerta) can be taken with or without food.

Do not crush, chew, or break an extended-release tablet. Swallow the pill whole. It is specially made to release medicine slowly in the body. Breaking the pill would cause too much of the drug to be released at one time.

If you have trouble swallowing the extended-release capsule, you may open the capsule and sprinkle the medicine into a spoonful of applesauce to make swallowing easier. Swallow this mixture right away without chewing. Do not save the mixture for later use. Discard the empty capsule. The chewable tablet form of methylphenidate must be chewed before you swallow it. To prevent sleep problems, take this medication early in the day, no later than 6:00 pm.

Store methylphenidate at room temperature away from moisture and heat.

Methylphenidate may be habit-forming and should be used only by the person it was prescribed for. Keep track of how many pills have been used from each new bottle you receive. Methylphenidate is a drug of abuse and you should be aware if any person in the household is using this medicine improperly or without a prescription.

What happens if I miss a dose?

Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at your next regularly scheduled time. Do not take extra medicine to make up the missed dose.

What happens if I overdose?

Seek emergency medical attention if you think you have used too much of this medicine. An overdose of methylphenidate can be fatal.

Symptoms of a methylphenidate overdose may include vomiting, agitation, tremors, muscle twitching, seizure (convulsions), confusion, hallucinations, sweating, fast or pounding heartbeat, blurred vision, dry mouth and nose, and fainting.

What should I avoid while taking methylphenidate?

Methylphenidate can cause side effects that may impair your vision or reactions. Be careful if you drive or do anything that requires you to be awake and alert.

What are the possible side effects of methylphenidate?

Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.

Stop taking methylphenidate and call your doctor at once if you have any of these serious side effects:

 

bulletfast, pounding, or uneven heartbeats;
bulletfeeling light-headed, fainting;
bulletincreased blood pressure (severe headache, blurred vision, trouble concentrating, chest pain, numbness, seizure);
bulletfever, sore throat, and headache with a severe blistering, peeling, and red skin rash;
bulletaggression, restlessness, hallucinations, unusual behavior, or motor tics (muscle twitches); or
bulleteasy bruising, purple spots on your skin.

 

Continue taking methylphenidate and talk with your doctor if you have any of these less serious side effects:

 

bulletvision problems;
bulletmild skin rash;
bulletdizziness;
bulletnervous feeling, sleep problems (insomnia);
bulletnausea, vomiting, loss of appetite; or
bulletweight loss.

 

Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome.

What other drugs will affect methylphenidate?

Before taking methylphenidate, tell your doctor if you are using any of the following drugs:

 

bulletblood pressure medications;
bulleta blood thinner such as warfarin (Coumadin);
bulletclonidine (Catapres);
bulletseizure medicine such as phenytoin (Dilantin), phenobarbital (Luminal), primidone (Mysoline); or
bulletantidepressants such as amitriptyline (Elavil, Etrafon), amoxapine (Ascendin), citalopram (Celexa), escitalopram (Lexapro), clomipramine (Anafranil), desipramine (Norpramin), doxepin (Sinequan),fluoxetine (Prozac, Sarafem), fluvoxamine (Luvox), imipramine (Janimine, Tofranil), nortriptyline (Pamelor), paroxetine (Paxil), protriptyline (Vivactil), sertraline (Zoloft), or trimipramine (Surmontil).

 

If you are using any of these drugs, you may not be able to use methylphenidate, or you may need dosage adjustments or special tests during treatment. There may be other drugs not listed that can affect methylphenidate. Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor.

What are tiers
 
What are "tiers", and how do they affect what I actually pay at the pharmacy?

Prescription medications are categorized within three tiers. Each Tier is assigned a copayment (or coinsurance percentage), which is an amount you pay when you fill a prescription at a participating retail pharmacy or refill your ongoing prescription through the network mail-order pharmacy service. Your employer or health plan sets the actual copayment amounts for the medications covered under your pharmacy benefit. Consult the benefit plan documents provided by your employer or health plan, or visit "Price a medication" on this Web site for more specific information about the copayments, coinsurance, and deductibles that may apply to your pharmacy benefit coverege.

Three-Tier Plans

Your Lowest Cost Option
Tier 1 is your lowest copayment option. For the lowest out-of-pocket expense, you should always consider Tier 1 medications if you and your doctor decide they are appropriate for your treatment.

Midrange Cost Option
Tier 2 is your middle copayment option. Consider Tier 2 medications if you and your doctor decide that a Tier 2 medication is appropriate to treat your condition.

Your Highest Cost Option
Tier 3 is typically your highest copayment option. Sometimes there are alternatives available in Tier 1 or Tier 2. If you are currently taking a medication in Tier 3, ask your doctor whether there are Tier 1 or Tier 2 alternatives that may be appropriate for your treatment. Compounded medications, those medications containing one or more ingredients that are prepared "on-site" by a pharmacist, are classified at the Tier 3 level, provided that the individual ingredients used in compounding are covered under the pharmacy benefit.

Two-Tier or Four-Tier Plans

Some plans have a two-tier or a four-tier pharmacy benefit rather than a three-tier pharmacy benefit. Generally, a two-tier closed pharmacy benefit plan does not cover medications classified in Tier 3 of the PDL. A two-tier open pharmacy benefit plan covers one tier at the lower copayment and covers a second tier at a higher copayment. For four-tier plans, Tier 4 is your highest cost option. You and your doctor make decisions about your treatment, including which medications are best for you.

The tier placement of a medication on the Prescription Drug List (PDL) may change. While medications change tiers infrequently, such changes can occur up to four times per calendar year, depending on your benefit. When a medication changes tiers, you may be required to pay more or less for that medication. These changes may occur without prior notice to you. However, you can find up-to-date information about tier status, copayment and coinsurance amounts for a particular medication by selecting "Price a medication" on this web site.

OMAHA, -- U.S. researchers say a clinical trial shows a nicotine vaccine can help people quit smoking and produces  
antibodies against nicotine. NicVax, made by Nabi Biopharmaceuticals, was tested on 301 study participants in nine cen-  
ters across the country, the University of Nebraska Medical Center said Wednesday in a news release. Dr. Stephen Rennard who presented the study results at the 80th American Heart Association Scientific Sessions in Orlando, Fla., said there was a significant relationship between anti-nicotine anti-body levels and continuous abstinence from smoking. He said  
NicVAX was well-tolerated and showed no differences in adverse events or in local/systemic reactions between placebo and each active vaccine group. Researchers said the vaccine works by preventing nicotine from entering the brain, which  
may reduce the pleasurable effects of smoking.

OTC drugs may cut Parkinson's disease risk  

LOS ANGELES, -- A U.S. study suggests over-the-counter medicines called non-steroidal anti-inflammatory drugs, or  
NSAIDs, may reduce a person's risk of Parkinson's disease. The study involved 579 men and women, half of whom had Parkinson's. The participants were asked if they had taken aspirin and if they had taken non-aspirin NSAIDs, such as ibuprofen, once a week or more at any point in their life for at least a month. The study found regular users of non-aspirin NSAIDs reduced their risk of Parkinson's disease by as much as 60 percent compared with non-regular users and  
non-users. Women who were regular users of aspirin reduced their risk of Parkinson's disease by 40 percent. "Our find  ings suggest NSAIDs are protective against Parkinson's diease, with a particularly strong protective effect among regular users of non-aspirin NSAIDs, especially those who reported two or more years of use," said study author AngelikaWahner of the UCLA School of Public Health. "Interestingly, aspirin only benefited women. It may be that men are taking lower doses of aspirin for heart problems, while women may be using higher doses for arthritis or headaches."

AUGUSTA, Ga.,--U.S. scientists have discovered environmental  
stresses can result in cancer development by reducing the  
activity level of an enzyme that causes cell death. Researchers led by Yonghua Yang, a postdoctoral fellow at the Med-  
ical College of Georgia Cancer Center, found stress-inducing  
agents, such as oxidative stress, recruit a protein called  
SENP1 that cuts a regulator called SUMO1 from the enzyme  
SIRT1 so its activity level drops. Yang said that finding  
opens the door for treatments that increase SENP1 activity,  
making it easier for cells that are becoming cancerous to  
die. "This is one of the things that make cancer cells so  
durable, one way they survive so well," said Yang. "We want  
to see if we can block that process and make cells die." He  
noted increased SIRT1 activity -- routinely present in cancer -- even makes cancer cells more resistant to anti-cancer  
drugs such as chemotherapy.
